For the majority of us, our independence is the cornerstone of our lives. We appreciate the level of care required to stabilize a medical condition and then we are ready to go home. For people with disabilities, this desire is no different. However, it may be necessary to adapt their home to their medical condition.
The Capital Area Center for Independent Living, in partnership with the State of Michigan offers the opportunity for individuals residing in nursing homes to transition back to their homes and families.
Under the supervision of a Rehabilitation Registered Nurse, our transition team will:
- Provide a nursing assessment to determine supports and accommodations.
- Develop a person-centered plan based on the desires of the individual.
- Find appropriate and affordable housing.
- Locate resources to modify housing and secure adaptive technology.
- Identify transportation options.
- Develop independent living skills (financial, nutritional, and personal advocacy)
- Establish community supports to promote success.
This service is at no cost to the client. (Client must have or be eligible for Medicaid)
